Go to Reports → Billing → Authorization Summary. Set the date range to a single closed billing month. Click Export → CSV.

Then go to Reports → EVV → Visit Detail for the same date range. Export as CSV. That’s it. Two files, both go here.
For claims: if you use WellSky Billing, add the Claim Status Report from the same Billing menu. Not required. The Margin Review runs without it and flags missing claims.

In AxisCare go to Reports → Authorization Report. Choose a completed billing period. Download as CSV.

Then go to Reports → Scheduling → Service Detail for the same period. Download as CSV. Drop both files here.
AxisCare exports vary by version. If you don’t see these exact menu items, look for "Authorization" and "Visit" under any Reports section. The column names are what matter, not the report name.

Log into HHAeXchange and go to Reports → Authorizations. Filter to one closed month. Export as Excel (.xlsx) or CSV.

HHAeXchange often exports as .xlsx, and that works fine here. You don’t need to convert to CSV.

In AlayaCare go to Reports → Funding → Authorization Utilization. Set date range to a closed billing period. Export as CSV.

Then go to Reports → Scheduling → Visit Report for the same dates. Export. Both files go here.
AlayaCare sometimes puts authorizations and visits in one combined report. If yours does, just upload that single file and we’ll pull what’s needed from it.
